Support Coordination

Support Coordination is an essential service that helps individuals with disabilities understand and effectively utilize their National Disability Insurance Scheme (NDIS) plan. Our team of experts works with individuals to identify the best service providers to meet their unique needs and coordinates a variety of supports, including mainstream, informal, community, and funded options.

We offer a range of support coordination services, including short-term assistance for implementing NDIS plans, assistance with building capacity to navigate services and supports, and understanding how to meet NDIS goals by connecting with appropriate service providers. Additionally, we provide a specialist level of support coordination by qualified Allied Health or Nursing Professionals to assist during times of crisis, complex circumstances, or during a transition period. With our support, individuals can fully understand and utilize their NDIS plan to achieve their goals and improve their quality of life.
